People who receive intensive help to lower high blood pressure, such as medication and exercise, have a lower risk of developing dementia, researchers have found.

According to the World Health Organization, dementia will affect 57 million people worldwide in 2025.

However, experts have long emphasized that dementia is not an inevitable consequence of aging.

Researchers have found that about half of cases can be prevented or delayed by eliminating 14 risk factors, including hearing impairment, smoking, obesity, excessive alcohol consumption and social isolation, and high blood pressure.

Researchers now claim that eliminating the last of these could reduce the risk of developing dementia by 15%.

The authors of the paper in Nature Medicine, researchers from China and the US, said the study involved 33,995 people aged 40 and older with uncontrolled high blood pressure who lived in 326 villages in rural China.

The team randomly selected 163 of these villages, and 17,407 participants who lived there received intensive blood pressure treatment from non-physician health care providers, often referred to as “village doctors.”

This included free or low-cost blood pressure medication prescribed in individualized doses, medical counseling to help adhere to medication and lifestyle changes - such as weight loss, alcohol and salt intake - and equipment and instructions to control blood pressure at home.

The remaining 163 villages, with 16,588 participants, received “usual care” - that is, participants' blood pressure was monitored in their usual clinical setting. Although participants were advised to make lifestyle changes and some were taking blood pressure-lowering medications, this group did not receive free home blood pressure monitors, medications, or instructions.

When the researchers followed up with the participants after four years, they found that 668 people in the intensive blood pressure monitoring group had dementia, while 734 people in the usual care group had dementia, with the analysis showing that the risk of dementia was 15% lower in the former group. Further work showed that this group had a 16% lower risk of cognitive impairment without dementia.
