Researchers from Johns Hopkins University in the US have found that people with diabetes have an accelerated loss of brain white matter (tissue containing nerve fibers). The results of the study are published in JAMA Network.

The study, which lasted 27 years, involved 185 people. Most were white Europeans with a college degree and a family history of dementia. The volunteers gave the scientists access to their medical records and consented to multiple MRI brain scans.

During the follow-up period, 60 participants developed mild cognitive impairment (MCD) and eight were diagnosed with dementia. The results showed that the risk of LCD was 86% higher in people with more white matter (nerve fiber) loss.

Further analysis showed that the amount of white matter in the brain was significantly reduced in those with type 2 diabetes. This led to a 41% increased risk of LCR.

The presence of dementia biomarkers in the cerebrospinal fluid increased the likelihood of cognitive problems by 50%. Patients with diabetes and evidence of amyloid plaques (neuron-damaging protein aggregates) at the same time were at highest risk. They were up to 55% more likely to develop mild cognitive impairment.

The scientists hypothesized that the body's inability to use insulin properly accelerates the accumulation of amyloid protein in the brain. This could trigger the development of Alzheimer's disease and lead to a more rapid transition from normal brain health to mild cognitive impairment.
